Кальций и болезни: гипертония, кальцификация органов и шок против энергии дыхания

Наиболее активно изучаемым в биологии и биохимии веществом является кальций, а потому примечательно, что исследователи все еще говорят о нескольких его парадоксах. Когда кости теряют кальций, мягкие ткани кальцифицируются; когда меньше кальция поступает с пищей, может вырасти кальций в крови и во многих органах и тканях; если орган — например, сердце — на короткое время лишить кальция, то его клетки теряют способность давать на кальций нормальный отклик и начинают поглощать его в больших, отравляющих количествах. 

Дефициты и магния, и кальция дают ряд сходных симптомов (к примеру, спазмы), но магний во многих системах является антагонистом кальция. Он является основным защитным блокатором кальция.

Воспалительный процесс ведет к чрезмерному поглощению кальция клетками и является фактором ожирения, депрессии и дегенеративных заболеваний.

В управлении метаболизмом кальция и в деле предотвращения остеопороза важную роль играют витамины К, Е и А. Аспирин (с его противоэстрогенными и подобными витамину Е свойствами) защищает от резорбции костей и гиперкальциемии.

Из всех регуляторных молекул наиболее изучен кальций, поэтому не удивительно, что у него более одного парадокса. Впрочем, существует такой подход к организму, в фокусе которого находится энергетический метаболизм, и нет необходимости привлекать специальную теорию кальциевых насосов.

Ионизированные атомы и молекулы ведут себя упорядоченно в зависимости от их размера и электрического заряда. Органическая материя, даже мертвая, избирательно связывает одни ионы металлов и выделяет другие. Живой организм вырабатывает поток метаболических продуктов, таких как двуокись углерода и молочная кислота, которые преимущественно взаимодействуют друг с другом и с ионами металлов, изменяя их концентрацию в клетках и телесных жидкостях. Такое ионное перемещение можно назвать активным транспортом, не прибегая к таинственным механизмам мембранных насосов. Химические изменения, вызванные в  клетке, например, дыханием, создают в отсеках тела разные электрические заряды (например, внутри и снаружи капилляров), которые по законам простых физических процессов, а не с помощью молекулярных насосов, управляют движением воды и перемещением ионов.

Результатом этих пассивных и активных процессов является то, что в соответствии с метаболическим энергетическим состоянием организма каждый вид иона имеет характерную концентрацию в том или ином отсеке.


Магний и калий в основном являются внутриклеточными ионами, а натрий и кальций — внеклеточными. Когда клетки возбуждаются, переходят в состояние стресса или обесточиваются, они теряют магний и калий и потребляют натрий и кальций. Митохондрии во время стресса могут связывать определенное количество кальция, но его накопление может достичь уровня их инактивации, что заставит клетки обратиться к неэффективному гликолитическому энергопроизводству и приведет к избыточной выработке молочной кислоты. Аномальная кальцификация начинается с митохондрий.

Клетки, находящиеся в состоянии напряжения или в процессе гибели, потребляют кальций, который, как правило, возбуждает и одновременно препятствует их энергопроизводству, усиливая стресс. Спазм или судорога являются примером неконтролируемого клеточного возбуждения. Длительное возбуждение и стресс способствуют воспалению тканей и фиброзу.

Вслед за фиброзом, который возникает при воспалении, обычно развивается обширная кальцификация.

Артерии, почки и другие органы в процессе старения кальцифицируются. В возрасте 90 лет количество кальция в эластичном слое артерии примерно в 35 раз больше, чем в возрасте 20 лет. Почти все ткани, включая мозг, чувствительны к воспалительным процессам, которые ведут от фиброза к кальцификации. Исключением является скелет, который теряет свой кальций, поскольку его поглощают мягкие ткани.

Эти наблюдения приводят к ряду простых представлений о природе старения и болезней.

К росту кальция в мягких тканях приводят также кислородная депривация, отравление цианидом, рентгеновское облучение и все другие виды травм.

Один из коллег Селье, Г. Джазмин, показал, что дефицит магния вызывает воспаление. Дефицит кальция или магния может стимулировать выработку паращитовидными железами больше гормона (паратиреоидного гормона, ПТГ), что усиливает поглощение кальция, но одновременно выводит кальций из костей. Этот гормон чувствителен к дефициту пищевого кальция или магния и является важным фактором, вынуждающим клетки потреблять слишком много кальция, а его избыток связан со многими воспалительными и дегенеративными заболеваниями.

Считается, что растущий при старении воспалительный цитокин интерлейкин-6 (ИЛ-6) играет важную роль в многочисленных процессах атрофии пожилого возраста. Одним из факторов, способных усилить производство ИЛ-6, является паратиреоидный гормон (ПТГ), который увеличивает количество циркулирующего в крови кальция частично за счет выведения его из костей; а ИЛ-6 стимулирует процесс вывода кальция из костей.

Интересны определенные взаимодействия между гормонами и другими регуляторными химическими веществами, хотя обычно к ним относятся как к частям машины, которая работает согласно скрытой программе, записанной в генах. Пролактин, растущий под действием эстрогена или серотонина, заставляет организм терять кальций (вытягивая его из костей) и стимулирует секрецию ПТГ, который компенсирует потерю кальция за счет его мобилизации из костей. Действие пролактина на кости, по крайней мере частично, осуществляется за счет роста ИЛ-6; ИЛ-6 стимулирует выработку пролактина. Серотонин и ИЛ-6 стимулируют секрецию друг друга, ПТГ и серотонин тоже стимулируют выработку друг друга.

ПТГ (как эстроген и серотонин) подавляет клеточное дыхание и активирует гликолиз, снижает уровень АТФ и сдвигает клеточный метаболизм в сторону выработки молочной кислоты, а не углекислого газа. ПТГ также приводит к потере бикарбоната с мочой.

Поскольку образование двуокиси углерода снижает внутриклеточный рН, а образование молочной кислоты повышает его (за счет реакции НАДН с пируватом), белки в клетке из-за недостатка кислорода или под влиянием этих гормонов становятся более отрицательно заряженными. Положительно заряженный ион кальция поглощается цитоплазмой клетки с высоким рН и увеличенным отрицательным электрическим зарядом. Кальций может поступать из относительно концентрированной внеклеточной жидкости, но также может быть высвобожден из кислых внутриклеточных хранилищ, аналогично выделению серотонина при нарушении рН.

Существуют и другие провоспалительные вещества, например, цитокины, которые оказывают такое же влияние на энергетические структуры клетки.

Известно, что антиметаболические свойства ПТГ имитируют антиметаболические эффекты при старении и диабете, а удаление паращитовидных желез устраняет диабет. ПТГ может вызывать диурез, приводя к уменьшению объема крови и обезвоживанию, гипертонии, параличу, ускоренному делению клеток и росту хряща, кости и других тканей.

Если просто употреблять достаточное количество кальция и магния, то можно облегчить многие считающиеся серьезными проблемы, связанные со стрессом и старением, такие как сердечная аритмия, панкреатит и кальцификация тканей. Противовоспалительные, антиаллергические свойства кальция и магния хорошо изучены, и есть очевидные доказательства того, что ожирение и различные эмоциональные нарушения могут быть результатом их дефицита. Хронически высокий ПТГ может вызывать анемию различными способами.

Поскольку диета с очень низким содержанием натрия увеличивает потерю магния, усиливая синтез альдостерона, простая добавка натрия к рациону может помочь некоторым людям сбалансировать минералы в организме и свести стресс к минимуму. Во время голодания и других интенсивных стрессов для поддержания способности выводить кислоты почки разрушают большое количество белка, образуя аммиак, поэтому повышенное употребление щелочных минералов может снизить белковый катаболизм.

Питание молоком и фруктами или молоком и мясом обеспечивает пищевой баланс с изрядным количеством кальция и магния. Листовые овощи — очень богатый источник магния, но они также являются потенциальным источником большого количества свинца и других токсинов. В 1960 году многие люди, включая членов правительства США, выступали за преимущественно вегетарианскую диету для детей из-за присутствия радиоактивного стронция в молоке. Я провел соответствующее сравнение с содержанием стронция в овощном рационе, способном снабдить организм адекватным кальцием и белком, в результате чего мне стало очевидно, что овощи являются наихудшим источником радиоактивного стронция, поскольку отношение стронция к кальцию в овощах намного больше, чем в молоке. Многим младенцам дают заменители молока (напитки здорового питания), изготовленные из сои или риса, и последствия ужасны. Если эти же продукты употребляют взрослые, то в краткосрочной перспективе последствия менее катастрофические, но свою роль в развитии дегенеративных процессов и деменции они скорее всего сыграют.

Большая часть внутриклеточного магния образует комплексное соединение с АТФ и помогает стабилизировать эту молекулу. Если внутриклеточное энергопроизводство невелико, как при гипотиреозе, клетки, как правило, очень легко теряют свой магнийвыделяя фосфат и смещая равновесие к молекуле с меньшей энергией, АДФ. АДФ образует комплексы с кальцием, а не магнием, увеличивая внутриклеточное содержание кальция.

Повышенный внутриклеточный кальций, в сочетании с избытком оксида азота и возбуждающими аминокислотами, участвует в нескольких нейродегенеративных заболеваниях, включая БАС, болезнь Альцгеймера, болезнь Паркинсона, хорею Хантингтона и эпилепсию. Известно, что магний, никотин, прогестерон и многие другие вещества защищают от эксайтотоксической кальциевой перегрузки, но в здравоохранении не наблюдается никаких последовательных усилий для рационального применения имеющихся знаний.

Дыхание и углекислый газ являются основными антагонистами ПТГ. При рождении у ребенка практически нет ПТГ, вероятно, из-за высокой внутриутробной концентрации углекислого газа, но в течение нескольких дней ПТГ повышается.

Повышенный углекислый газ способствует образованию костей, а пониженный бикарбонат способствует потере кальция костью (Canzanello, et al., 1995; Bushinsky, et al, 2001). Употребление бикарбоната натрия может стимулировать образование костей.

Диета с низким содержанием белка, подобная тому, как питается значительная часть женщин (0,8 г/кг массы тела), увеличивает ПТГ и, вероятно, способствует развитию остеопороза и заболеваний кальцификации. При крайней белковой недостаточности происходит сдвиг в сторону воспаления, избытка серотонина и чрезмерного свертывания крови, что может быть связано с действием более мягкой, более распространенной белковой недостаточности. Многие люди отстаивают диету с низким содержанием белка, в частности, для профилактики или лечения остеопороза, но культуры, которые традиционно едят много белка, такие как масаи, очень здоровы. Недавние исследования (см. Bell and Whiting, 2002) подчеркивают важность животного белка в профилактике остеопороза.

Традиционные мясоеды эффективно употребляют в пищу все животное, включая кровь, кожу, кости и различные органы, а не только мышцы. Прогестерон, который увеличивает содержание углекислого газа в тканях, способен замечательно подавлять действие большинства медиаторов воспаления и катаболизма, защищая от дегенеративной кальцификации и остеопороза. Он также защищает от аномального свертывания крови. ПТГ повышает концентрацию кальция в тромбоцитах и при некоторых условиях может привести к неадекватной коагуляции.

Аспирин ингибирует действие ПТГ, помогая предотвратить кальцинирование воспаленных тканей и препятствует потере кальция костью. Аспирин снижает выработку ИЛ-6.
Белок, называемый ПТГ-родственным белком (ПТГрБ), имеет те же функции, что и ПТГ, но может вырабатываться в любой ткани. Он отвечает за гиперкальциемию рака и, по-видимому, участвует в частых метастазах рака молочной железы в кости.

Лишь немного изменив теоретическое представление о природе живого организма, осознав важность взаимодействия метаболитов и структурных веществ, управляемых энергетическим метаболизмом, можно было бы добиться реального прогресса в понимании болезней и здоровья. Самый важный парадокс кальция — это продолжающаяся пропаганда медицинскими журналами (например, International J. of Cardiology, Dec., 2002) идеи о том, что слишком много кальция вызывает затвердевание артерий и других заболеваний кальцификации.

All rats were fed a 0.8% calcium diet. Urinary calcium excretion (mg/period, mean +/- SEM) was increased during both week 1 and week 2 (16 +/- 3 vs 9 +/- 1 and 16 +/- 2 vs 9 +/- 1, CO2 group vs controls, respectively [p < 0.05]). Net intestinal calcium absorption (intake minus fecal excretion) was increased throughout the period of hypercapnia (week 1, 213 +/- 19 mg vs 135 +/- 15 mg; week 2, 135 +/- 16 mg vs 43 +/- 14 mg; and cumulatively, 344 +/- 27 mg vs 178 +/- 20 mg, CO2 group vs controls [p < 0.01]). As a consequence of the marked increment in intestinal calcium absorption during hypercapnia, mean net calcium balance was more positive than that of controls throughout the study (week 1, 197 +/- 18 mg vs 126 +/- 15 mg; week 2, 120 +/- 15 mg vs 34 +/- 15 mg; and cumulatively, 317 +/- 25 mg vs 159 +/- 20 mg, CO2 group vs controls, respectively [p < 0.01]). The serum ionized calcium concentration did not change at night in either group. Humans enjoying exceptional longevity on earth are especially susceptible to calcium deficiency in old age. Low calcium and vitamin D intake, short solar exposure, decreased intestinal absorption, and falling renal function with insufficient 1,25(OH)2 vitamin D biosynthesis all contribute to calcium deficiency, secondary hyperparathyroidism, bone loss and possibly calcium shift from the bone to soft tissue, and from the extracellular to the intracellular compartment, blunting the sharp concentration gap between these compartments. The consequences of calcium deficiency might thus include not only osteoporosis, but also arteriosclerosis and hypertension due to the increase of calcium in the vascular wall, amyotrophic lateral sclerosis and senile dementia due to calcium deposition in the central nervous system, and a decrease in cellular function, because of blunting of the difference in extracellular-intracellular calcium, leading to diabetes mellitus, immune deficiency and others (Fig. 6)

Conditions associated with immune dysfunction such as aging, corticosteroid therapy, and rheumatoid arthritis are associated with osteoporosis, which is also more common in females than in males, like most of the autoimmune-collagen diseases. Peripheral lymphocyte subsets CD4/CD8 were higher in patients with senile osteoporosis than in the age-matched controls, and returned to normal after 1 month of 1 alpha(OH)vitamin D3 treatment. Plasma interferon reflecting macrophage function decreased with advance in age and increased in response to 1 alpha(OH)D3 treatment. As one of the immunoregulators, vitamin D tends to stimulate the macrophage-natural killer system and suppress the lymphocyte system, stimulating TGF beta and TNF alpha activity. Senile osteoporosis of low turnover thus appears to be associated with vitamin D deficiency, low macrophage function, high CD4 lymphocyte proportion, low IL-1 and high IL-2 activity, low IFN alpha and high IFN gamma activity, and low TGF beta and TNF alpha activity.

Fujita T Calcium is unique in its distribution in living organisms with an extremely high hard and soft tissue and extra- intracellular concentration gradient. Calcium deficiency through stimulating parathyroid hormone secretion tends to blunt such a difference by paradoxically increasing the calcium concentration in the soft tissue and intracellular compartment. Since aging is associated with theprogressive aggravation of calcium deficiency, such blunting also progresses with aging. The dysfunction, damage and death of cells occurring in all diseases is always associated with a blunting of the extra- and intracellular calcium components. Calcium supplement especially with highly biologically available active absorbable calcium, was associated with the suppression of parathyroid hormone secretion and the normalization of a such blunting of intercompartmental distribution of calcium examples in hypertension and diabetes mellitus with evident improvement of clinical manifestations and laboratory tests.

Hypovitaminosis D is frequently observed in Europe in the elderly, particularly in the institutionalized population, but is also seen in otherwise healthy younger adults. An estimated 40% of the young European population has some degree of hypovitaminosis D. Finally, the beneficial effect of moderate sun exposure on cutaneous vitamin D synthesis (and psychological well-being) must not be overlooked.

Parathyroid hormone-related protein (PTHrP) is a ubiquitous and highly conserved vasoactive peptide whose role and regulation in normal physiology remain an enigma. Recently, we demonstrated that low-dose endotoxin (LPS) induces intrasplenic, but not systemic, levels of PTHrP; and that tumor necrosis factor, a pro-inflammatory cytokine, is the major mediator of this effect. We have therefore hypothesized that, with higher, lethal doses of endotoxin, PTHrP could be induced in multiple tissues to such a degree that it could contribute to the lethality of septic shock. In response to a near-lethal dose of endotoxin, PTHrP mRNA levels increased acutely in every vital organ examined (spleen, lung, heart, kidney, and liver). Circulating levels of PTHrP also increased, peaking 2 hr after administration of high-dose endotoxin. These results suggest that PTHrP belongs to the cascade of pro-inflammatory cytokines induced during lethal endotoxemia that is responsible for the toxic effects of LPS.

Previous studies in our laboratory have demonstrated that cell-mediated immune function was suppressed in female, but not male, mice at 10 days after burn injury and was mediated, in part, by increased production of interleukin-6 (IL-6). Increased circulating concentrations of E(2) corresponded with suppressed delayed-type hypersensitivity (DTH) and splenocyte-proliferative responses, and increased circulating concentrations of IL-6 in female mice after burn. Ovariectomy restored the suppressed DTH response and decreased IL-6 concentrations, and administration of exogenous E(2) to both ovariectomized females and intact male mice resulted in a suppressed DTH response. In addition, in vitro treatment with E(2) suppressed splenocyte proliferation in a macrophage-dependent manner and enhanced macrophage production of IL-6.

The study demonstrates oscillation of serum IL-6 concentration during the MC and the marked MC-dependent modulation of IL-6 secretion by sex hormones.

The increase in overall calcium intake due to different drinking water induced modest increases in calcium excretion, whereas oxalate excretion tended to decrease. The changes in oxalate excretion during any one study period compared with another were significantly related to those in calcium intake. Citrate excretion was significantly higher with the high-calcium, alkaline water. 4. Parathyroid hormone, calcitriol and markers of bone resorption increased when patients were changed from the high-calcium, alkaline to the low-calcium drinking water. 5. We suggest that overall calcium intake may be tailored by supplying calcium in drinking water. Adverse effects on bone turnover with low-calcium diets can be prevented by giving high-calcium, alkaline drinking water, and the stone-forming risk can be decreased as effectively as with low-calcium drinking water.

For any level of urinary sodium, hypertensives excreted more calcium. These preliminary data suggest that parathyroid gland function may be enhanced in essential hypertension.

Hypercapnic acidosis caused little change in PGI2 production, but hypocapnic alkalosis increased it markedly.

Reducing extracellular pH from 7.35 to 6.60, or reducing ambient temperature from 37 degrees C to room temperature, also attenuated the increases in steady-state pH(i) observed during and after anoxia and reduced rates of pH(i) recovery from acid loads imposed in the immediate postanoxic period. The results suggest that a Zn(2+)-sensitive acid efflux mechanism, possibly a H(+)-conductive pathway activated by membrane depolarization, contributes to the internal alkalinization observed during anoxia in adult rat CA1 neurons. The rise in pH(i) after anoxia reflects acid extrusion via the H(+)-conductive pathway and also Na(+)/H(+) exchange, activation of the latter being mediated, at least in part, through a cAMP-dependent signaling pathway.

When pH(o) was altered, the effects on force and calcium were the same irrespective of whether force had been produced spontaneously by high-K depolarization or by norepinephrine; alkalinization increased tone, and acidification reduced it. Because pH(o) changes also lead to changes in pH(i), we determined whether the effects on force could be explained by these induced pH(i) changes. It was found, however, that only with spontaneous activity did intracellular alkalinization increase force. In depolarized preparations, force was decreased, and, with norepinephrine, force was initially decreased and then increased. Thus the effects of pH(o) cannot be explained solely by changes in pH(i). The role of the sarcoplasmic reticulum (SR) and surface membrane Ca(2+)-ATPase on the mechanism were investigated and shown not to be involved. Therefore, it is concluded that both pH(o) and pH(i) can have powerful modulatory effects on portal vein tone, that these effects are not identical, and that they are likely to be due to effects of pH on ion channels rather than the SR or plasma membrane Ca(2+)-ATPase.

Since arachidonic acid and other polyunsaturated fatty acids influence the activities of most ion channels, we studied their effects on the erythrocyte Ca(2+) influx. AA (5-50 microM) and EPA (20-30 microM) stimulated a concentration-dependent increase in [Ca(2+)](i), deriving from extracellular calcium (1 mM), without affecting the intra- and extracellular pH and membrane voltage. We conclude that AA could activate an erythrocyte voltage-independent Ca(2+) transport via an intermediate product of cyclooxygenase pathway...

By means of fura-2 microspectrofluorimetry, we measured the intracellular calcium concentration ([Ca2+]i) of OHCs bathed in Hanks' balanced salt solution (pH 7.40) during either a resting state or high K+-induced depolarization. While gentamicin and extracellular acidification (pH 7.14) can separately attenuate this increase in [Ca2+]i in both types of OHCs, their suppressive effects are additive in basal OHCs, but not in apical OHCs.

The biologically active fragment of human parathormone (PTH) and intact bovine PTH were found to induce secretion of both serotonin and histamine from rat peritoneal mast cells in vitro. Intradermal injection of PTH induced immediate increases in vascular permeability suggesting that PTH could induce mast cell secretion in vivo. These results demonstrate that elevated levels of PTH can induce mast cell secretion in vitro and in vivo and suggest a possible role for mast cells in the pathophysiology of non-allergic disease states.

The baseline levels of serum estrone were significantly higher in all of the patients than in age-matched normal subjects. Serum baseline testosterone, LH and FSH levels were all essentially normal, except low FSH levels in Duchenne muscular dystrophy.

Relation of magnesium to osteoporosis and calcium urolithiasis. Wallach S Magnesium influences mineral metabolism in hard and soft tissues indirectly through hormonal and other modulating factors, and by direct effects on the processes of bone formation and resorption and of crystallization (mineralization). Its causative and therapeutic relationships to calcium urolithiasis (CaUr) are controversial despite an association between low urinary Mg and CaUr. Recent studies have also found a tendency to low serum and/or lymphocyte Mg levels in CaUr. Despite earlier studies demonstrating an inhibitory effect of Mg supplementation on experimental CaUr in animals and in spontaneous CaUr in humans, at least two properly controlled clinical trials of Mg supplementation have failed to demonstrate a beneficial effect on CaUr frequency. With regard to the skeleton, experimental studies have shown that Mg depletion causes a decrease in both osteoblast and osteoclast activity with the development of a form of 'aplastic bone disease'. At the same time, bone salt crystallization is enhanced by Mg deficiency. Conversely, Mg excess impairs mineralization with the development of an osteomalacia-like picture, and may also stimulate bone resorption independently of parathyroid hormone. Whether or not Mg depletion may be a causal factor in human osteoporosis is also controversial, and there are conflicting reports as to the Mg content of osteoporotic bone. Small decreases in serum and/or erythrocyte Mg in osteoporotic patients have been reported, and one author has noted improved bone mineral density with a multinutrient supplement rich in Mg. The extant data are sparse and indicate a clear need for more rigorous study.

The hypothesis that the Ca and Mg deficiency in areas with soft drinking water increases the risk of I.H.D. death rate was supported by the finding that food loses more Ca and Mg when it is cooked in soft water as compared to cooking in hard water. However, investigation of a group of 17 municipalities of which mortality and water quality data are known for three periods, 1958-1962, 1965-1970 and 1971-1977, showed that the inverse statistical relation between I.H.D. Mortality and water hardness still existed but with decreasing significance of correlation coefficients.

It activates osteoblasts, increases bone mineralization, and enhances the sensitivity of target tissues (incl. bone) to PTH and 1,25(OH)2 vitamin D3, Under certain conditions however, magnesium can stimulate bone resorption. A more potent factor than magnesium is stroncium, which not only activates osteoblats but decreases the number of osteoclasts, thus abolishing bone resorption and enhancing formation. Bicarbonates are also favourable for bone. NaHCO3 together with potassium citrate stimulates osteoblasts and enhances bone mineralisation.
